Millions of American men are reportedly using testosterone replacement therapy (TRT) without full awareness that it can compromise their fertility. Experts warn that the widely accessible treatment, often obtained without a direct physician consultation, carries significant risks for future family planning.
Understanding Testosterone Replacement Therapy Risks
Testosterone replacement therapy, or TRT, is used by an estimated 11 million men in the United States. These men seek the treatment for a range of reasons, including muscle enhancement, improved bone health, increased focus, and combating fatigue. A primary driver for its use is the perceived improvement in sexual health, addressing low libido, sexual performance issues, and erectile dysfunction.
As men age, natural testosterone levels decline. TRT is a common prescription for those who struggle to produce sufficient testosterone. However, urologists emphasize that certain men should avoid it, even when accessible.
Impact on Sperm Production and Fertility
Dr. Justin Dubin, a urologist and director of men’s sexual health at Herbert Wertheim Cancer Institute, stated that "Taking testosterone kills your sperm and causes infertility in men." While it does not literally kill sperm, TRT significantly reduces sperm production. This occurs because the body detects sufficient testosterone and ceases its own production. Without the necessary hormonal signals, sperm generation halts.
Within three to six months of consistent use, many men stop producing sperm entirely. The return of sperm production after discontinuing TRT can take months, up to two years, and is not guaranteed. Some individuals may experience permanent infertility.
Dr. Alexander Pastuszak, a urologist and CEO of Paterna Biosciences, noted that the "tubes that are no longer being used as often to release hormones 'shut down and shrink.'" This can lead to noticeable testicular atrophy within months of consistent TRT use.
Potential Health Concerns and Proper Usage
TRT is generally considered safe when testosterone levels remain within a normal range and under close medical supervision. This necessitates regular monitoring by a physician to detect any adverse effects. Historically, concerns have existed regarding TRT's potential to increase the risk of heart attacks and prostate cancer.
Recent research has led to the rollback of some restrictions, with some studies suggesting TRT does not increase heart event risks. Regarding prostate cancer, TRT does not cause it but can accelerate the growth of existing tumors. This highlights the importance of cancer screening and medical supervision for patients undergoing TRT.
Concerns also arise when testosterone is used without a genuine medical need. One study indicated that only 12% of men on TRT actually met the criteria for a prescription. Without proper medical oversight, excessively high testosterone levels can lead to adverse effects.
Recognizing and Addressing Underlying Issues
Abnormally high testosterone levels, exceeding 1000 nanograms per deciliter, can result from incorrect dosing or the patient's inability to metabolize the drug effectively. Dr. Dubin recommends physician assessments and blood work evaluations every three to six months for TRT patients.
Elevated testosterone can cause increased red blood cell production, leading to higher blood pressure and strain on the heart. It can also overstimulate sebaceous glands, causing acne. Breast development, or gynecomastia, can occur through aromatization, a process where excess testosterone converts to estrogen, stimulating breast tissue growth.
TRT may mask underlying health problems rather than solve them. For example, treating low testosterone associated with sleep apnea without addressing the apnea can worsen it. Similarly, relying on TRT for weight loss or blood sugar control without lifestyle changes may prove ineffective.
Obtaining TRT online bypasses essential medical consultations, hindering an accurate health assessment. Low testosterone is often linked to other health issues such as poor diet, lack of sleep, sedentary lifestyles, obesity, diabetes, and chronic diseases.
Dr. Dubin suggests alternative treatments like clomiphene and enclomiphene, as well as human chorionic gonadotropin (hCG). These alternatives can potentially increase testosterone levels while preserving or improving fertility.
